Troop 628 Forms
Permission Slip: Each scout must have a completed permission slip and emergency contact form on each outing. Generic Permission Slip Form
Account Form: This form is used by scouts to withdraw funds from the boy scout account. Money may be accumulated in a fund for a scout in many ways: purchasing script, fund raisers such as Christmas Wreath sales or delivering the monthly Lake Forest Beach And Tennis Club newsletter. This money may be applied to summer camp, monthly campouts or outings and to purchase boy scout supplies such as uniforms, and camping supplies.
Scouts no longer registered with Troop 628 must use these funds within six months for BSA related expenses. Funds may be used for reimbursement of scouting purchases only. Funds will not be refunded to you in cash or transferred to another troop. Funds not used within six months of failing to re-register with Troop 628 revert to the Troop 628 general fund.
Scrip Form: We have a wonderful way for you to save money for the activities that your child participates in while in our troop. SCRIP is gift certificates that you can purchase from different businesses, which in turn give a percentage to your child’s scout account. For example: STORE-A gives a 9% return. If you purchase $100 in STORE-A Scrip, 8% of $100 or $8 will be added to your boy's scout account. The troop takes 1% for costs.
These funds may be used for Troop outings, summer camp and other Boy Scout functions. If you purchase Boy Scout clothing at the BSA Scout Shops, keep your receipt and get reimbursed from your boy's scout account.
To use the Boy Scout Account funds, you must fill out a "Scout Account Withdrawal Form".
